how to apply:

Who is Eligible?

As of February 1st, scholarship applications are available for seniors graduating from high school in 2012. To be eligible, you must be a Salem resident attending any high school and have financial need.

How to Apply

The Dollars for Scholars application contains three very important sections. In each section you need to pay close attention to details, ensuring that all questions are answered accurately and completely. Please note: Incomplete and late applications, financial assistance questionnaire and/or update sheets will not be considered.

Section 1: Four-page application [Click here to Download]
Due: Thursday, February 9, 2012 by 2:30 PM to Mrs. Fazioli in the Guidance Department at Salem High School

Fill out each section of the application, including all requested information. There is an Applicant Appraisal on the last page, this can be left blank if you wish to have your guidance counselor complete it. However, if you want a teacher to complete the appraisal it must be done prior to the deadline. The completed application must be turned into Mrs. Fazioli at the Salem High School Guidance Department by 2:30 PM Thursday, February 9, 2012. Please do not drop off the application without speaking to Mrs. Fazioli she needs to give you additional information. At that time, a file is prepared, using an ID number in place of the applicants name to ensure confidentiality.

Section 2: The Financial Assistance Questionaire (FAQ) [Click here to Download]
Mailed: By Friday, March 12, 2012

This form is absolutely necessary to complete the application process. It must be completed and mailed with a $5.00 check, made payable to DOLLARS FOR SCHOLARS by Monday, March 12, 2012 to:

Scholarship America - Dollars for Scholars
One Scholarship Way
St. Peter, MN 56082

Section 3: Update form will be mailed from the guidance department to the applicant's home in March. If you do not receive one, please stop by the guidance department and pick one up.

The information on the Financial Assistance Questionnaire is analyzed and collated at the National Headquarters of Citizens Scholarships Foundation of America, Inc., in Minnesota, where the amount of the parent's expected contribution is determined and then forwarded back to the Guidance Department at Salem High School.

The Guidance Department then prepares a file for each student applicant, removing any family names and assigning an ID number to maintain the anonymity of the applicant. These files are then turned over to the Selection Committee for their use with the information contained on the application form.

All recipients will be notified by mail in May by the Dollars for Scholars Committee.

If you have any questions, please call your student's guidance counselor.
